By submitting this form, you agree to the following:

• Recipient must live within Randolph County or the Ohio side of Union City.
• Recipient must not already have a usable bed
• Parent(s)/Guardian(s) of recipient must be in financial need of help/assistance, willing to stand before God and be judged
• A second bed for a recipient may not be requested, even if the recipient lives in multiple homes
• The request for a bed(s) may be denied at any point in time
